Facility Evaluation Report
Licensing Program Analyst, LPA, Noel Wolf Petersen arrived unannounced to conduct an annual inspection on 10/8/25. LPA met with administrator Amber Griffen-Thomas and explained the purpose of the visit. Linden Grove Adult Residential is a 6 capacity facility with a current census of 5. They have 5 ambulatory, 0 nonambulatory, and 0 bedridden clients. There are 0 restricted conditions in the facility, for which they have no presented exceptions and waivers.
The physical plant inspection included but was not limited to: bedrooms, bathrooms, common areas, storage areas, exteriors, and evacuation routes. The facility traffic areas are unobstructed. Kitchen has functional hardware, the pantry has adequate food storage space. LPA observed some boxes of foods outside of their expiration date. TV was issued. bathrooms had functional hardware, deliver water within the 105-120*F. Bedrooms have adequate furniture and furnishings, including bed encasements. medicine, toxics, and cleaning supplies have adequate lockable storage. common areas and exteriors have furnishings in good repair and are free of triping hazzards. Emergency exit is unobstructed. Required posters are present and of the correct dimension.
