Facility Evaluation Report

Announced office visit was conducted, via Teams, on 07/18/2025 by the following participants at this time:

Jessica Dizon, facility designated Administrator

The purpose of this office meeting was to review the current Informal Compliance plan for this facility which was originally conducted on 06/14/2024. It was agreed upon that the facility Licensee/designated Administrator would be responsible for the following at that time:

Have a certified facility Administrator on duty at least 20 hours per week at each facility for Schumard Care Home and Schumard Care Home 2

Quarterly visits will be made out to each facility

TSP referral, if agreed upon, by the facility designated Administrator

This facility was determined to have been in compliance for the past year and has been removed from the Informal Compliance process at this time.

There were no deficiencies observed or cited during today's office meeting.

A copy of this report will be sent, via email, to the facility designated Administrator to sign and return a copy back to this LPA. Exit Interview
