The inspector’s account

Licensing Program Analyst, LPA, Noel Wolf Petersen, arrived on 11/20/25 at 12:30pm, to conduct an annual inspection. Currently there are no clients in care as the administrator is waiting on vendorization, and some elements are out of compliance as a result. LPA is asking for specific changes before clients are accepted into care. LPA met with administrator Boluwatife Adeoti, to explain the purpose of the visit.

Physical Plant was inspected including but not limited to the bedrooms, bathrooms, exteriors, common areas, kitchen, evacuation routes and storage areas. bedrooms have all required furniture, LPA gave guidance about plastic encasements for the mattresses. Bathrooms have functional hardware, the water temperature was measured within 105-120*F. Administator provided fire extinguishers were serviced at some point in the last year, she will provide some kind of paper evidence of that to noel.wolfpetersen@dss.ca.gov. Common areas are clean, well lit, and unobstructed. Exterior is not clean and has many trip hazzards and transversal obstructions in the form of weeds. Administrator provided that she has sought a quote, LPA asked for a picture of the backyard, clean and free of trip hazards and obstructions to be sent within the next two weeks, by end of day 12/6/25. Exterior evacuation gate has a lock on it, LPA asked for the lock to be removed before a resident moves in. Kitchen has lockable storage for toxics, medication, and sharps. Food is not adequate for 2 days perishable and 7 days non perishable, for 4 clients, (162 meals). LPA gave guidance that more staple dry food(rice, beans) could make up the difference until clients can express preferences.

No clients interviewed, No staff interviewed, no records reviewed for staff or clients, Administrator files were checked: infection control plan, evacuation plan, control of the property, surety bond, liability insurance, are present and up to date.

No citations issued, the licensee will tell the LPA when clients have been accepted into care. a copy of the report was read and given to the licensee.
