This is an official report of an unannounced visit/investigation of a complaint received in our office on

11/26/2024 and conducted by Evaluator Kesha Lewis

COMPLAINT CONTROL NUMBER: 27-AS-20241126101855

On 02/25/25, Licensing Program Analysts (LPA) Kesha Lewis and Nole Wolfpeterson arrived unannounced to deliver findings for the allegations noted above. LPA met with As taff and explained the purpose of the visit.

LPA lewis was unable to concact R1 to confirm the allagetions and interviews with staff and other residents did not indicate that and actions had occured. Note that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, and therefore the allegations are unsubstantiated.

An exit interview was held and a copy of this report was given.
