Facility Evaluation Report

Licensing Program Analysts's (LPA) Melina Oropeza and Kesha Lewis arrived at this facility unannounced to conduct a plan of correction visit . LPA's were met by staff and administrator joined 15 minutes later. LPA's explained the purpose of the visit to Administrator and staff.

LPA's visited the facility to address the plan of correction was not received by the due date 03/11/2026. Civil penalties are being issued during this visit and will continue to be assessed at $100 per day until the physicians report (LIC 602) is received by the department for R1. See the LIC 421F form from the visit on 03/10/2026.

Exit interviewed conducted. Copy of the report was left with the administrator.
