Facility Evaluation Report

On January 26, 2024, Licensing Program Analyst (LPA) Avelina Martinez arrived announced to conducted a Pre-Licensing Inspection of the facility to ensure compliance with Title 22 regulations. LPA Avelina Martinez met with Staci Stevens, Darin Hassig, and Leslie Springer, who assisted LPA Martinez in today’s inspection.

Facility has a capacity for 16 clients. The fire clearance is for 12 ambulatory clients and 4 non-ambulatory clients. Rooms 1, 2, 3, and 4 are allowed four clients per room. Each room has a fire clearance for one non-ambulatory client. In addition, the facility has an approved SRF waiver to increase occupancy of four (4) bedrooms. The occupancy of four (4) bedrooms from two (2) clients to four (4) clients. Additionally, this SRF is a six (6) month program.

Staci Stevens will be the Administrator of this facility. The facility administrators certificate is current and expires on 03/12/2024. Staci Stevens is currently working on submitting their Administrator recertification application to the Department.

This facility is located next to Tuning Point community programs Sage Village 1, which both facilities will have Staci Stevens appointed as the Administrator. Sage Village 1 and 2 have the same facility layout.

LPA Martinez toured the facility with the following individuals: Staci Stevens, Darin Hassig, and Leslie Springer. LPA Martinez inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms and bathrooms, and dinning room.

During today's inspection, the facility was sanitary and all common areas were furnished. Resident bedrooms and bathrooms were furnished and sanitary. The facility has a locked room for medication storage. In addition, the facility has a first aid kit, which is complete. The facility had a fire inspection, and the facility was cleared. The facility fire extinguishers and carbon detectors are in good repair. The facility has a December 28, 2023 approved fire clearance.
