Facility Evaluation Report
Licensing Program Analyst (LPA) T. White made an unannounced visit on this day for the purpose of conducting a Case Management, Health and Safety visit. LPA met with Administrator, Elizabeth Abesa and explained the purpose of the visit.
Present at time of visit were 12 residents and 3 staff members. LPA inspected the interior and exterior of the facility. LPA verified that the facility had running water and electricity. LPA tested water temperature in the resident's bathrooms which measured 119.1 degrees F.
LPA inspected food supplies and observed two days worth of perishable and seven days worth of non-perishable foods.
An exit interview was conducted with Administrator and a copy of this report provided.
