Facility Evaluation Report

On 5/26/23 at approximately 2:30pm Licensing Program Analyst (LPA) Maja Jensen arrived at facility unannounced to conduct a 1 year annual visit. LPA Jensen met with Administrator Jessica Owens and Compliance Manager Alex Archangel and explained the purpose of today's visit. Jessica holds current Administrator certificate # 6055006735 good through 01/07/2024.

LPA Jensen toured the facility and the grounds. Due to time constraints this annual inspection will require a continuation.

An exit interview was conducted and a copy of this report was provided.
