The inspector’s account

Licensing Program Analyst(LPA), Noel Wolf Petersen arrived unannounced to conduct a case management investigation reffering to an incident where a resident was reported to be running away from the facility, saying "hes going to get me". LPA met with Facility Manager Casey Kwok, and later Administrator Alex Archangel to explain the purpose of the visit.

The physical plant was inspected, full details on attached 812.

The facility provided medical records from the preadmission assessment and a recent discharge, the diagnosis from the recent discharge broadly fits the symptoms presented during the incident as remarked upon by staff in direct connection to the incident. self described histories provided by resident 1 in the preadmission assessment also broadly fit the incident as remarked upon by staff in direct connection to the incident. Without coming into contact with the resident as they have moved on from the facility, the LPA is receptive to the claim made that the incident appeared to be made as a result of expressing symptoms, unrelated to a real person at the facility or in the community. no other residents interviewed made a claim that a staff or another resident was a safety concern to them.

LPA provided guidance that if there is an observation of a change in status by the staff of a resident, that incident should make it to the attention of a relevant professional for assessment of any changes to care.

No citations were issued, a copy of the report was read and given to the staff.
