The inspector’s account

On 7/1/25 at 10:00 am Licensing Program Analyst(LPA) Noel Wolf Petersen, arrived unannounced and met with Jessica Gutierrez at G.L.O.M.4(Glom4) and explained the purpose of the visit: to conduct an annual inspection. Glom4 is a 15 bed social rehabilitation facility, current census is 15, all ambulatory.

The Physical Plant was inspected, including but not limited to the resident bedrooms, bathrooms, common areas, hallways, storage, exteriors and evacuation routes. The facility is clean, the traffic areas are well lit and unobstructed. The bedrooms have required furniture and furnishings, including dressers, lighting, personal item storage, and bedding encasements. Bathrooms are in good repair and have required furnishings including covered trash cans. The administrator provided that the clients are supported by documentation to handle soaps and other topically-applied hygienic materials. Common areas are clean, furniture and activities are age appropriate and in good repair. Storage has adequate supply of linen, sharps and cleaning supplies are away from clients in care, and emergency food for a 2-days of perishable and 7-days of nonperishable food for 15 people exists. Medications are stored locked, LPA checked the medication and MARs for 2 clients at random and found the documentation to support the inspected drugs in kind and quantity. The fire extinguishers are dated 1/30/25. The exterior of the plant is in good repair, with some small quantity of food trash(tinfoil/solo cup/etc.), the administrator removed the trash immediately. Of the Required Posters (facility sketch with evacuation route, federal work rights, Personal Rights, Ombudsman contact) 2 are present; the administrator provided that the walls are recently painted and put up the required posters in the common area/staff area.
