Facility Evaluation Report

On 06/16/2025, L PA Yi Sam Jian conducted an unannounced case management inspection in response to an incident report submitted by the facility to the Regional Office (RO), concerning a physical altercation between two residents. As a result of the incident, one resident was transported to the emergency room, and the other was arrested by the San Francisco Police D epartment (SFPD). The LPA met with Darnell Williams, Administrator and explained the purpose of the visit.

The LPA conducted an interview with the administrator. The two staff members who witnessed the incident were unavailable at the time of the visit. The resident who was arrested has been released from the facility, and the resident taken to the emergency room declined to be interviewed. The LPA obtained copies of the residents’ records.

The LPA reviewed this report with the administrator and provided a copy for their records.
