Facility Evaluation Report
On 06/16/2025 Licensing Program Analyst(LPA) Yi Sam Jian arrived at the facility to conduct the Annual 1-year required visit. LPA was greeted by Darnell Williams, Administrator and explained the purpose of the visit.
LPA toured facility and grounds of this 24 bedroom Adult Residential Facility. There are 12 rooms on the main level and 12 rooms on the 2nd level. No accessible bodies of water or fire safety hazards are observed. Most rooms accommodate 2 persons. Washer and dryer are located in lower level staff/conference room. There is no garage. There is a large front yard and no backyard. There are two delayed egress gates on the street, which are in in good working order. Food supplies are adequate. Medications, toxins and sharps are stored appropriately and inaccessible to clients, a comfortable temperature is maintained, and client rooms are lighted by ceiling lights. Soap and paper towels are present in bathrooms and kitchen sink, including reminder signs to wash hands. Shower curtain lowered to prevent water from leaking out of shower stall. First-aid kit is inspected and complete. Criminal record clearances or exemptions for facility staff or other individuals who have client contact have been reviewed, as well as other staff records and first-aid training.
