Facility Evaluation Report

On 07/13/2026, Licensing Program Analyst (LPA) Yi Sam Jian conducted an unannounced case management visit. LPA were greeted by staff Wayne Smith and explained the purpose of the visit.

Based on observation interview and record review, deficiency is cited under California Code of Regulations, Title, 22 cited on the LIC 809D. Failure to correct the deficiencies may result in civil penalties.

This report is reviewed and discussed with staff. A copy of this report and the appeal rights were provided. Due to multiple deficiencies cited, information about the CCLD Technical Support Program is recommended and provided to Wayne Smith, to provide assistance and training in meeting regulatory requirements. Staff reported the facility agree to request TSP services, and LPA will facilitate referral to TSP assistance.
