Facility Evaluation Report
Licensing Program Analyst (LPA) Melissa Parks arrived on Wednesday August 5, 2026, to conduct the unannounced annual inspection.
During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPA reviewed client (2) and staff (5) files. All client and staff files contained the required paperwork.
LPA Parks and staff toured the facility together to ensure the health and safety of clients in care. The areas toured included bedrooms, kitchen, bathrooms, medication room and outside space. Carbon monoxide detectors were observed. Facility was current on fire drills. First aid kit had all required components. Fire extinguisher had current inspection tag. Water temperatures were within the required range. In the areas toured, there were no health or safety violations observed.
No deficiencies cited. Exit interview conducted. A copy of this report was provided to the facility.
