Facility Evaluation Report
Licensing Program Analyst (LPA) Melissa Parks arrived on Wednesday September 17, 2025 to conduct an announced annual inspection.
During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPA Parks reviewed Client (1) and staff files (4). Client file contained the required paperwork. Staff files contained the required paperwork and training.
LPA Parks and staff toured the facility together to ensure the health and safety of clients in care. The areas toured included client rooms, bathrooms, living room, kitchen, staff office and backyard. In the areas toured, there were no health or safety violations observed.
The facility was clean and well organized. All required postings were observed. All knives/sharps were kept locked and inaccessible to clients. First Aid kit was fully stocked. Facility had full supply of perishable and nonperishable foods. Facility was current with fire drills.
No deficiencies cited. Exit interview conducted. Due to technical issues, this report was emailed to the Administrator after the visit was completed.
