Facility Evaluation Report
Licensing Program Analysts (LPAs) Talwinder Bains and Lavinia Muscan arrived at the facility unannounced on 07/02/25 to do case management visit. LPAs met with administrator, Sheri Kimbro and explained the purpose of the visit.
Department followed up on SOC341 sent by the facility on 06/30/25. SOC indicated that resident, R1 reported that on 06/28/25 around 5:00 AM, R1 was forced to take their morning medications (against their will) by staff, S1. R1 reported that they refused to took their medication, but S1 forcefully gave the medication and shoved it in R1s mouth. It was learnt that S1 was terminated from facility on 06/30/25 due to this incident.
Department conducted interviews with residents and staff regarding this incident during today’s visit.
LPAs determined from record review, staff, and resident interviews that facility took appropriate measures to address this incident and reported this incident to all relevant parties as required.
This incident is under review and Department will follow up further as needed.
Citation was issued per Title 22 Regulations as indicated on LIC-809D.
Exit interview conducted, appeal rights and copy of the report left at facility.
