Facility Evaluation Report

Licensing Program Analyst (LPA) Talwinder Bains arrived at the facility unannounced on 05/22/25 to conduct a case management to follow up on SOC341 related to residents. LPA met with facility Administrator Sheri Kimbro and explained the purpose of the visit.

The facility submitted a completed SOC341 on/around March 18, 2025 regarding incident which happened on/around November 26, 2024. Per SOC341, staff S1 was terminated after it was reported to management on/around November 26, 2024, S1 was verbally abusive to residents while on duty. Based on record review and staff interview, it was concluded that facility did not report this incident to Department in timely manner as required per reporting requirements.

Based on this information, violations are cited today p er California Code of Regulations, Title 22, Division 6, Chapter 8. Deficiencies issued are noted on the LIC809D.
