Facility Evaluation Report
On 07/01/25, Licensing Program Analysts (LPAs) Talwinder Bains and Lavinia Muscan arrived unannounced to conduct a case management inspection following receipt of incident report (LIC624) for a resident on 06/18/25. LPAs met with Administrator, Sheri Kimbro and explained purpose of inspection.
LPAs interviewed Administrator and discussed death of resident (R1) in more detail, including resident's recent condition prior to passing. LPAs requested R1s file documents to be sent to LPA Bains via email by 07/02/25 by 5PM.
At this time, further analysis is needed, and a copy of death certificate is also requested for review.
There are no deficiencies being cited during today's inspection for this incident.
