Facility Evaluation Report

Licensing Program Analyst (LPA) Sarena Keosavang arrived at the facility unannounced on 05/04/2022 to conduct a Case Management- Health Checks. LPA met with Executive Director (ED), Chad Rogers, and explained the purpose of the visit. LPA wore a Surgical Mask and were screened by facility staff upon entering the facility.

LPA toured the facility together with ED to ensure health and safety of residents in care. Areas toured include but are not limited to: common areas, four (4) resident bedrooms and one (1) bathroom. In the areas toured no immediate health, safety, or personal rights violations were observed. Facility was found to be in substantial compliance at this time.

No deficiencies are being cited as a result of todays inspection.
