Licensing Program Analyst (LPA) Michael Hood arrived at the facility and met with Resident Care Director (RCD), Sharisse Toves, to conducted a case management health and safety check.
Prior to visit, LPA received a death report for resident (R1). LPA obtained and reviewed documentation pertaining to R1. LPA will conduct a follow-up visit if deemed necessary.
No deficiencies are being cited as a result of today's visit. Exit interview was conducted. Signature on these forms acknowledges receipt of these documents.
