On 10/7/25, LPA Kevin Mknelly conducted a case management visit and met with caregiver. Administrator was notified and arrived to assist.
The purpose of the visit was for a health and safety check.
LPA observed 6 residents and reviewed resident files.
LPA spoke with administrator and reviewed records and services plans for residents.
As a result of today's inspection no deficiencies were found.
