Facility Evaluation Report

Licensing Program Analyst (LPA) Bethany Mirlohi arrived unannounced to conduct a case management visit. LPA met with Nicoleta Hegseth during today's inspection.

LPA arrived today to complete a facility tour due to a new fire clearance granted to facility on 8/3/26. Fire clearance stated Bedroom 1 and 2 are approved for staff. Bedrooms 3, 4, 5, 6 and 8 are approved for Non ambulatory and bedroom 7 is approved for a bedridden resident. LPA toured facility with administrator. LPA requested an updated LIC999 (Facility Sketch) and LIC610E.

No deficiencies cited during today's inspection.
