On 5/28/26, LPA Mknelly attempted and Annual inspection. No answer to doorbell. Facility appears to continue to be vacant. LPA called facility contact phone number and left a voice message. LPA will reschedule this inspection.
LPA returned for a scheduled visit on 6/24/26 as the residence has no residents in care.
LPA and administrator conducted a inspection of the facility.
The home is clean and in good repair. All rooms had required furniture and fixtures. Administrator agreed to notify LPA if residents are admitted to this location.
