On 6/5/25, LPA Mkelly attempted and annual inspection. The home appeared vacant. LPA attempted to reach the Administrator by the phone and email addresses on file before leaving the premises. LPA will reach out to the Admin to schedule a visit.
LPA Mknelly confirmed that there are currently no residents in care.
LPA and administrator conducted a inspection of the facility.
The home is clean and in good repair. All rooms had required furniture and fixtures. Administrator agreed to notify LPA if residents are admitted to this location.
