On 9/11/25, at 9:40 AM, LPA Mknelly returned to amend a report previously written on 9/9/25. Licensing Program Analyst (LPA) Kevin Mknelly, conducted a plan of correction visit while and met with Edward Estante , caregiver.
On 8/26/25, the department issued two citations in complaint findings. The citations had plans of correction (POCs) due by 9/2/25. LPA found that plans of corrections were submitted by email on 8/26/25.
Civil penalties issued, on 9/9/25, for the late POCs will be deleted.
As a result of today’s inspection, no additional deficiencies were noted.
