Facility Evaluation Report

Licensing Program Analyst (LPA) Ivan Avila and Licensing Program Manager (LPM) Lauren Crocker made an unannounced required 1-year annual inspection at the facility today on April 27, 2023. Met with caregiver Cindy Holman and Licensee Hazel Estante. During this visit a tour of the facility was conducted, a review of the resident and staff records was complete and a sample of the medications was reviewed. Due to time constraints an additional visit will need to be made to finalize this annual inspection.
