On 5/17/24 at 10:30am Licensing Program Analyst (LPA) conducted an unannounced Collateral inspection to address a reported incident for February 2024.
LPA conducted an interview with one participant regarding allegations that took place in their residential facility.
Exit interview conducted and a copy of this report was left at the facility.
