Facility Evaluation Report

On 05/20/26 at 4:00 pm, a virtual office meeting was held with Sacramento North Regional Office using Microsoft Teams.

Present in the meeting was: Licensee, Adeola Ogundiwin, Licensing Program Manager (LPM) Troy Ordonez, and Licensing Program Analyst (LPA) Graham Gunby.

The purpose of today's meeting was to discuss a pending change in the facility's ownership.

Also discussed is the facility's requirement to issue a notice to all residents and their responsible person(s) advising of the pending change in ownership, and how a copy of that notice needs to be submitted to the Department within (5) days of its issuance.

LPM emphasized how communication needs to be ongoing with the Department during the ownership transition, and for any other significant changes, such as a change to the property ownership, and the facility's first point of contact is the assigned LPA.

LPM requested copies of the Lease Agreement and Change of Ownership notification given to residents by 05/22/2026.

A copy of this report was emailed to Adeola Ogundiwin, Licensee, following today's meeting. A signed copy will be returned to the LPA by COB 05/22/26. There are no deficiencies issued in this report.
