Facility Evaluation Report

On 6/18/26, Licensing Program Analyst (LPA) Kevin Mknelly, conducted a case management visit while investigating a complaint and met with Admin/ designee.

While conducting complaint file reviews and interviews it was found that licensee had not followed through on agreed upon conditions set forth in a 5/20/26 office meeting for a change of ownership.

MJ Cabrera was not designated as the administrator. S2 and S3 were not verified to be cleared and associated.

MJ will submit transfer requests for himself and S3.

Citation and civil penalties for failure to designate an administrator and S2 lack of criminal record clearance.

S2 will move from the home until clearance is verified.

As a result of this inspection, the following deficiencies were cited on 809-D, per Title 22 Regulations, Division 6. (A)This poses an immediate Health and Safety risk to clients/residents in care. (B) This poses a potential Health and Safety risk, or personal rights violation, to clients/residents in care.
