03/16/2023 and conducted by Evaluator Bethany Mirlohi

COMPLAINT CONTROL NUMBER: 59-AS-20230316092544

Licensing Program Analyst (LPA) Bethany Mirlohi arrived unannounced to deliver complaint findings for allegation listed above. LPA met with Antonette Edwards during today's visit.

During the investigation it was determined that the alleged allegations were prior to licensure of this facility. These allegations do not apply to the current Licensee therefore, the department has determined that complaint is unfounded. Exit interview was conducted with Administrator and a copy of this report was provided to the facility. The signature of Administrator on these forms acknowledges receipt of these documents.
