On 8/19/25, Licensing Program Analyst (LPA) Kevin Mknelly, conducted a case management visit while delivering complaint findings and met with Administrator .
On 8/8/25 the department received an incident report regarding R1. R1 had a fall with injuries.
LPA received requested records and conducted interviews.
As a result of today’s inspection, no deficiencies were noted. LPA will continue to gather additional information.
