Facility Evaluation Report

On 12/04/2025 Licensing Program Manager (LPM) Troy Ordonez and Licensing Program Analyst (LPA) Bethany Mirlohi met with Licensee Kulwinder Kaur, and potential new licensee Suzette Bole via teleconference to discuss change of ownership of facility.

Suzette Bole has submitted a new application for new ownership of facility. Licensee understands during this process of change of ownership she will remain the licensee and that her license is not transferable.

Licensee to submit into LPA form LIC308 in order to change Administrator to Suzette Bole once purchase of property and business is completed.

Licensee to ensure Suzette Bole is fingerprint cleared and associated to the facility.

Licensee to submit a copy of the lease back agreement once the purchase of the property and business has been completed.

LPA will forward a copy of this report to licensee. Licensee to review report, sign, and return a signed copy to CCL.
