Facility Evaluation Report
Licensing Program Analysts (LPAs) Lavinia Muscan and Lavinia Talwinder Bains arrived at the facility unannounced on 5/28/25 to do a case management visit. LPAs met with Administrator, Loredana Pop and explained the purpose of the visit.
Incident : Department conducted a case management visit to follow up on an incident which happened on 5/15/25 as reported by the facility. Incident report stated that: on May 15, 2025, at approximately 5:30pm, the caregiver initiated a self-cleaning function on the oven. Shortly after, staff noticed smoke coming from the oven. Staff immediately began evacuating residents and 911 was called. All residents were assessed by paramedics and found to be in good health. Residents were relocated to nearby facilities for the night. Fire Department advised that once the house was properly ventilated and kitchen was cleaned from debris and new oven installed, it would be safe for residents to return to facility. Residents returned 5/17/25.
Facility notified all residents, families, responsible parties, and all other required agencies regarding this incident. Facility followed emergency protocol to handle this incident and conducted staff training.
All residents are back at the facility. No immediate health and safety concerns were identified related to residents during today’s visit.
No citations were issued per Title 22 Regulations.
Exit interview conducted and copy of the report left at facility.
