Facility Evaluation Report

Licensing Program Analyst (LPA) Bethany Mirlohi arrived unannounced to conduct a case management visit. LPA met with administrator James Dial during today's inspection.

LPA received 2 incident reports concerning medication errors. On 11/09/25 staff administered eye drops to R1 in error. Staff informed R1's PCP and responsible party. Resident had no adverse effects. On 11/22/25 it was found that R2 was receiving the wrong dosage of medication that was prescribed by R2's PCP. Responsible party and PCP were notified of the error. Resident had no adverse effects. Administrator stated that training was given to staff for both medication errors.

There are deficiencies cited during today's inspection. Deficiencies cited on the 809-D.
