On 9/25/25, Licensing Program Analyst (LPA) Kevin Mknelly, conducted a case management visit met with Administrator .
The purpose of the inspection was to follow-up on issues previously discussed regarding staffing, upkeep of the facility and activities for residents. At todays inspection, Administrator and a caregiver were present.
Several topics discussed. LPA and licensee will schedule a meeting to establish an action plan.
As a result of today’s inspection, no deficiencies were noted.
