On 5/12/26, Licensing Program Analyst (LPA) Kevin Mknelly, conducted a case management visit while delivering complaint findings and met with Administrator.
On 5/6/26 R1 experienced a fall while toileting independently. The fall resulted in a a leg fracture.
LPA reviewed records and received copies.
R1 medical assessment and care plan stated R1 transfers independently.
The fall was unwitnessed. 9-1-1 was called to assist.
As a result of today’s inspection, no deficiencies were noted.
