Licensing Program Analyst Hiratsuka, conducted this unannounced annual visit.

This facility has three shared resident rooms and two staff rooms. There is one full common bathroom and a private bathroom in one of the staff rooms. The second staff room is near the garage. There is a laundry room next to the garage. There is a door that connects the house to the garage. There is an ample supply of perishable and nonperishable food. There is a locked cabinet for medications in the dining room. There is a gate on the same side as the garage.
