The inspector’s account

Licensing Program Analyst (LPA) Kevin Mknelly arrived at the facility unannounced on April 3, 2025 to conduct a Annual Inspection utilizing the CARE inspection tool. LPA met with staff and explained the purpose of the visit. Administrator is present to assist.

LPA toured the interior of the facility together with staff to ensure health and safety of residents in care. Areas toured include but are not limited to: common areas, resident bedrooms, bathroom, kitchen, laundry room, and backyard. In the areas toured no immediate health, safety, or personal rights violations were observed. Water temperature exceeded regulation range. The home is very clean and residents interviewed stated they are happy with care.

LPA reviewed 3 resident files. LPA and Administrator discussed content and detail of records and new regulation updates.

LPA reviewed a staff files. File is complete. LPA and Administrator discussed having staff training records available for review.

LPA and Administrator held extensive discussed regarding regulation updates and issues related to pending change of ownership.

LPA received: copy of an LIC 500, LIC 308 and current Admin cert.

A deficiency are being cited as a result of todays inspection.

Exit interview conducted with Administrator and copy of report and appeal rights left at the facility.
