Facility Evaluation Report
Licensing Program Analyst (LPA) Bethany Mirlohi arrived unannounced to conduct a case management visit. LPA met with Charles Tinsley, Administrator and informed her the reason for the visit.
Alta Regional Center issued a Facility Action Report because of deficiencies observed during the annual Title 17 monitoring review on 11/03/2025. They were placed on sanctions because of deficiencies observed, and no respites or placements until the Corrective Action Plan is completed.
It was found that C1 was prescribed a medication daily for the PM. The MAR was reviewed and it showed C1 had not been receiving this medication since June 2025. LPA spoke to administrator in which they stated C1 has had this topical medication prescribed for 2 years. Facility ran out of the topical cream in June and made an appointment with dermatologist to get a new prescription. Administrator reports dermatologist office rescheduled the appointment several times before they were able to be seen on November 5th. They have now received a new medication and C1 is receiving the medication.
No deficiencies cited during today's inspection.
