The inspector’s account

Licensing Program Analyst (LPA) Cassandra Mikkelson and Licensing Program Manager (LPM) Laura Munoz arrived at the facility unannounced to conduct a case management visit. LPA and LPM met with Administrator Baby Quintero and explained the reason for the visit.

LPA and LPM toured the facility and conducted a review of resident medications and resident file review.

As a result of this visit, deficiencies were cited on 9099-D. Appeal rights given.
