Facility Evaluation Report
Licensing Program Analyst (LPA) Cassandra Mikkelson, conducted this unannounced case management visit in response to Case Management visits conducted on 05/28/2025 and 06/04/2025.
The following observations were cited on 05/28/2025. Civil Penalties were assessed on 06/04/2025 for below citations.
- 87465(h)(2) Incidental Medical and Dental Care- Facility did not meet regulations as medications were left unlocked in kitchen drawer.
- 87309 Storage Space and Access - Facility did not meet regulations as knives and chemicals were unlocked and accessible to residents.
During today's visit, LPA observed above deficiencies to be corrected and maintained. LPA and Licensee discussed the importance of maintaining the above regulations moving forward. LPA also reviewed five (5) resident records.
No deficiencies were cited during today's visit. Exit interview conducted with Licensee.
