Facility Evaluation Report

Licensing Program Analyst (LPA) Bethany Mirlohi arrived unannounced to conduct a case management visit. LPA met with Administrator Delson Bautista and explained the purpose of today's visit.

LPA arrived to review staffing schedules and staffing associations for the facility. Regional center found overlapping staff for Angel Cares Home 2, INC. and Angel Cares Home, LLC. Administrator stated they provided an old schedule to regional center that did not reflect the current schedule of the homes. LPA reviewed staff association lists and disassociated staffing list and found all staff have been associated that worked at the home. Administrator has updated the staffing schedules for both homes to reflect current staffing levels.

No deficiencies cited during today's inspection.
