Facility Evaluation Report

Licensing Program Analyst (LPA) Cassandra Mikkelson and Licensing Program Manager (LPM) Laura Munoz conducted a virtual meeting with BJ Thompson (Regional Center), Jona Diamond (Regional Center), Delson Bautista (Facility Administrator), Crisel (Facility Licensee), Dr. Johnson (Client C1's beahviorist), and Orlando Carpio (Facility Assistant Administrator).

LPA and LPM discussed Client C1 and their needs and services and what behaviors C1 has at the facility that is prompting Emergency Services to be called. As a group, it was discussed how the facility can redirect C1 when there are behaviors instead of calling 9-1-1 emergency services. Alta Regional Center and facility will work together to update C1's Individualized Program Plan (IPP) to indicate the steps for redirection.

Alta Regional Center and facility will also work with C1's conservator to create a plan that will help C1 during their behaviors.

LPA and LPM reminded facility to continue to report any incidents or behaviors to LPA for documentation purposes.

LPA sent hard copy to Administrator Delson Bautista for signature as meeting was virtual. LPA requested Administrator to send signed copy back to LPA for filing.
