Facility Evaluation Report

Licensing Program Analyst (LPA) Bethany Mirlohi arrived unannounced to conduct an annual inspection. LPA met with Administrator Delson Bautista during today's inspection. Currently the facility is waiting for referrals from the regional center, and therefore no clients reside in the home.

LPA toured the facility with administrator and observed the inside and outside of the home. LPA toured all client rooms, common areas, and kitchen area. Administrator agrees to inform LPA when first client moves into the home.

LPA provided administrator with a copy of report.
