Facility Evaluation Report

Licensing Program Analyst (LPA) Bethany Mirlohi arrived unannounced to conduct Case management. LPA met with Team Lead Wendy Centeio during today's visit.

LPA arrived at the facility to discuss incident reports that were received from the facility. LPA interviewed staff concerning incident. It appears facility followed proper protocol and regulation on each incident that occurred.

No deficiencies cited during today's inspection.
