Licensing Program Analyst (LPA) Bethany Mirlohi arrived to the facility and met with licensee Kathryn Quebada. Licensee has requested to close the facility. LPA toured the facility and observed no clients in the home. LPA obtained facility license and will close the facility as of 6/30/26.
