Facility Evaluation Report

Licensing Program Analyst (LPA) Bethany Mirlohi, (LPM) Troy Ordonez and (LPM) Laura Munoz arrived unannounced to conduct a Case management visit. LPM spoke to Licensee, Amitis Pourarian on the phone during today's inspection.

During the annual inspection conducted on 10/28/2025, Department staff observed this location is being operated as a martial arts studio, with signage and physical setup consistent with a commercial martial arts business. During today’s visit, LPM Munoz observed a business license issued by the City of Roseville licensing The Studio Martial Arts and Fitness.

LPMs Munoz and Ordonez spoke to the licensee on the phone. The licensee confirmed that no clients are currently enrolled or receiving services under the licensed Adult Day Program (ADP). Licensee reported that the Adult Day Program was initially licensed in 2016 and the program has remained inactive since licensure.
