Facility Evaluation Report

On 11/13/24, Licensing Program Analyst (LPA) Kevin Mknelly, conducted a case management visit to amend a previous report . Caregivers notified administrator of the visit and Anita Heydon arrived to assist.

LPA presented an amended report, written 6/20/24, to dismiss the citation that was issued at that time.

During today's visit, LPA observed 4 residents in care with two caregivers. Residents appeared to have their care needs met.

As a result of today’s inspection, no deficiencies were noted.
