Facility Evaluation Report
Licensing Program Analyst (LPA) Melissa Parks arrived on Thursday February 19, 2026 to conduct a case management visit regarding an incident and death report that was submitted to the Department.
LPA learned that R1 was found on the floor on 2/2/2026 next to their bed. Per R1, they went to the bathroom independently then lost consciousness. R1 was transferred to the hospital for evaluation. R1 was admitted to the hospital with a diagnosis of UTI and dehydration. While in the hospital, R1 developed pneumonia, developed a change in condition and passed away on 2/7/2026.
LPA obtained a copy of R1's care plan and physicians report. Per the care plan, R1 was independent with dining and eating, continent of bowel and bladder, and independent with mobility. R1's care needs were last assessed on 11/30/2025.
No deficiencies cited. Exit interview conducted. A copy of this report was provided to the facility.
