Facility Evaluation Report

Licensing Program Analyst (LPA) Melissa Parks arrived on Thursday February 5, 2026 to conduct a case management visit. LPA met with Krisztina and explained the purpose of the visit.

This facility has a fire clearance for 6 non ambulatory residents. R1 has a diagnosis of spastic quadriplegic cerebral palsy and is not currently on hospice. Although R1's physicians report and care plan state that R1 is non ambulatory, staff and R1 state that staff provide repositioning throughout the day and night. Therefore, R1 is considered bedridden.
